Please read and follow these steps carefully to be fully registered. IMPORTANT: You are registered once ALL steps are completed. |

Step 1: All students who plan to participate in athletics must have a physical examination within thirteen months of the start of each season. Per state law and the MIAA, physical examinations that expire result in immediate ineligibility until a new physical examination has taken place. Physical examinations must be performed by a duly registered physician, physician's assistant or nurse practioner. Athletes WILL NOT be allowed to participate without having a physical exam in good standing.
All documention should be submitted to the School Nurse and to the Director of Athletics. You can get the blank Physical Form here.

Step 2: Fill out and submit the Online Registration Form here (includes all forms in one).
Step 3: Once you complete these steps, you are registered and can fully participate in the Wareham Public Schools athletic program.
